Complex Care Intervention


Before completing an intake form, please review the funding options below. Select the one that best suits your situation, and follow the provided information and links to apply.

Funding Options:

  1. Social Worker or Agency Funding:
    If you seek funding through a social worker or agency, please refer to our CCI Flyer and forward it to your contact for funding approval. If you have any questions regarding funding, your social worker or agency can contact us directly.

  2. Post Adoption Assistance:
    If you qualify for post-adoption assistance through the Ministry of Children and Family Development (MCFD), your adoption social worker must submit a request on your behalf. They will require a description of the services provided by CCI and the associated costs. You can provide them with our CCI Flyer to facilitate the request. Once approved, you r your social worker can complete the intake form and contact us directly if you have any questions.

  3. Grant Options:

    1. Jordan's Principle:
      Available for Indigenous children or youth living on reserve with a parent who has status under the Indian Act. The child must be registered or eligible to be registered under the Indian Act and recognized by their nation for Jordan's Principle. Applications can be submitted by family members, agencies, or schools. The application and more information can be found at Jordan’s Principle. Please include the program name “Complex Care Intervention” by Complex Trauma Resources on page 6 of the application form. Include an appendix of detailed information by attaching our CCI Flyer to your application.

    2. 54.01 Kinship Care - Enhanced Out of Care Funding:
      If you are eligible for Enhanced Out of Care funding, please discuss this option with your social worker. Additional information on child-youth eligibility and the support agreement can be found here. Please attach our CCI Flyer to your application form for eligibility.

    3. Autism Funding:
      If your child has been diagnosed with Autism Spectrum Disorder (ASD) they may quality for professional services by CTR. More details are available here.

  4. Private Pay:
    We accept direct payments if you prefer to pay privately. Please ensure you are prepared to arrange payment when completing the intake form.

Please note that we do not accept insurance for direct billing.
